# 3. How to create a Survey

#### Prerequisites:

* [x] You have a Sleuth user account.
* [x] You've created your own personal Teamspace in [1. How to create a Teamspace](https://help.sleuth.io/pulse/beginner-tutorials/1.-how-to-create-a-teamspace).
* [x] You Sleuth administrator has enabled the [Slack integration](https://help.sleuth.io/pulse/beginner-tutorials/broken-reference) for Sleuth

To create a new Survey, complete the following steps:

1. [Sign in](https://app.pulse.sleuth.io/sign-in) to Sleuth.

2. Select your personal Teamspace (created in the prior tutorial) from Sleuth's left-hand navigation pane. The Teamspace displays with the **Reviews** tab is in focus.

3. From the left-hand navigation pane, click the **Surveys** tab under your Teamspace, then click **New survey**.

4. Click any **Survey template** to create a new draft Survey.&#x20;

5. Sleuth displays the new draft Survey.&#x20;
